Your Medical Records
Upon your admission, an electronic record is created under your name. It is used to manage your documents and files, ensure the billing of procedures and send healthcare requests requiring a coverage authorization to public and private third-party payers.
Your Right to Access Your Medical Records
Your attending physician is available to provide you with details and any information about your health.
Any doctor designated by you can also review your complete medical records, after having informed the attending physician.
You can obtain a "Discharge Order" written by your attending physician that provides a complete hospitalization summary.
Any request for a discharge order or duplicate must be made in writing, signed by yourself and presented to the hospital's Medical Records Department, along with a copy of your ID.
You can also obtain duplicates of imaging results at your expense.
